Registration Form (New-to-PUSD Students Only)

Registration is the final step to enroll your child in school.

Registration forms are currently accepted for the 2024-2025 school year.

Pre-enrollment (registration for the 2025-26 school year) begins on December 3, 2024.

Families may only complete registration forms for their child's neighborhood school or lottery-offered school.

Lottery applications are required for all transitional kindergarten programs, dual language immersion programs, and school transfer requests. Lottery applications must be submitted (and offers accepted) before registering.
Data Confirmation (Continuing PUSD Students)
- Pre-enrollment is no longer required for students who already attend PUSD schools. Instead, parents/guardians must create or log in to their existing Parent Portal account at the start of each school year to complete “Data Confirmation.”
- Continuing PUSD students' information will automatically remain at their current school until the exit grade level and roll over to the student's assigned neighborhood school after their current school's exit grade.
- If a continuing PUSD student participates in the lottery, the student's information will instead roll over to their lottery-offered school.
- Applications are required for all students requesting to transfer schools, including requests to leave a lottery/permit school to transfer to the student's neighborhood school for the next year.
- Transfer applications for the 2025-26 school year are accepted January 8, 2025 - August 1, 2025.
Required Enrollment Documents
Parent/Guardian Photo Identification (1 required)
- Driver’s license or CA ID card
- Government-issued ID with photo
- Military ID card
- Passport
- Bank or employee ID card with photo
Address Verification (1 required; must be dated within 60 days of submission and show the name and address of the parent/guardian)
- Property tax bill/receipt
- Current rental/lease agreement and rent receipt
- Utility service (gas, electric, water) contract, statement, or receipt
- Correspondence from a government agency
- Monthly mortgage statement or escrow document
Student's Birthdate Verification (1 required)
- Birth certificate
- Hospital record of birth
- Passport
- Adoption papers
- Baptismal certificate
Student's Immunization Record
- Must be up to date with California immunization requirements for TK-12th grade students
Health Forms (Submit completed forms to the school’s nurse/health clerk)
- TB Risk Assessment Questionnaire (all new students
- Health History Form (all students, updated annually)
- Health History Form (Spanish) (all students, updated annually)
- Oral Health Assessment Form (Transitional Kindergarten/Kindergarten)
- Oral Health Assessment Form (Spanish) (Transitional Kindergarten/Kindergarten)
- Report of Health Examination for School Entry (No longer required but recommended for Kindergarten/1st grade)
Other
- Parent Portal Access Form / Parent Portal Access Form (Spanish)
- Special Education Documents – Copy of most recent IEP; Please contact the special education department (new to PUSD): 626-396-3600 Ext. 88600
- Legal Documents – Foster placement, court documents, and/or restraining orders
- Permits to attend a school other than the student’s neighborhood school: School lottery offer, intradistrict permit, and/or interdistrict permit
- Transcripts or most recent report card for students previously enrolled in a non-PUSD school
